In this paper, you will explain the concept of relational tables and submit all of the relational tables for the database. Address the following:

1.Provide an introduction about the topic of the paper and how the paper will be organized. ER Chen diagram and relational tables for database plan for the problem:

Skilled Nursing Facility (SNF) has been experiencing an increasing number of returning patients to acute care before they are discharged.

2. Explain the concept of relational tables.

3. Update your Entity-Relationship diagrams from your Week 3 assignment incorporating any changes that were necessary to address your three questions; include the necessary entities, attributes including primary and secondary keys, and cardinality rules (i.e. properly drawn lines and symbols). If possible, please indicate which entities and/or attributes will utilize standard nomenclature. (see ER Chen diagram attached)

4. Show relational tables, each with a unique name, in standard relational-table format (see Relational Tables attached)

5. For each relational table, name attributes, along with at least one data characteristic for each attribute. This content can be presented in narrative form, as individual tables.

6. Explain how the tables relate to each other.

7. Identify the primary key and any secondary keys for each table. If secondary keys are used, provide a rationale for why they are used. (did NOT use secondary keys)

8. Conclude the paper with a recap of the content and insights gained from working on this assignment.

9. Include your three questions on your diagram: (see below)

• How many patients are readmitted to the acute care facility?

• What is the leading cause of patients being admitted to acute care despite being exposed to quality care?

• Why are most patients readmitted within a week of discharge?
