ASSIGNMENT OVERVIEW
This paper presents a database plan for a nursing or healthcare problem. You will explain the problem and the reason(s) a database is needed.
The Problem:
The main problem is the number of patients returning to the acute care facility before they discharge from the Skilled Nursing Facility (SNF) and after discharge.
Reason a database is needed:
The perfect database that needs to be accessed is the number of patients who access the facility and are admitted back into acute care. There is also a need to access data on the reason for returning to acute care.
After explaining the problem and need for the database, address the following database plan elements:
1. Provide a conceptual data model for the planned database. (see attached Chen ER diagram)
2. Name all the entities planned for the database and justify their selection.
3. Provide an E-R diagram that shows each entity, names the identifier for each entity, and illustrates the relationships between each entity, noting the cardinality.
4. Provide a narrative explanation of key parts of the E-R diagram, including how you arrived at some of the decisions on relationships and cardinality.
5. Include three questions planned for this database (the original questions from the discussion in Week 1 may be used).
3 questions are:
1. How many patients are readmitted to the acute care facility?
2. What is the leading cause of patients being admitted to acute care despite being exposed to quality care?
3. Why are the majority of patients readmitted within a week of discharge?
6. Conclude with a recap of the paper and insights gained on databases developed from working on this assignment.
7. Support all decisions with scholarly literature. 3 Scholarly sources no older than five years.
Introduction: Introduction includes information about the topic and the organization of the paper.
Problem: Problem is described, and reason for a database as an information-management solution is explained.
Conceptual model: Conceptual data model for the planned database is described.
Entities: All entities for the database are named.
Justification: Justification for each entity is provided.
E-R diagram: Each E-R diagram, provided separately for each entity or as a comprehensive diagram, is discussed briefly in text and found illustrated in an appendix.
Identifier: Identifier for each entity is named.
Relationships: Relationships between entities are discussed briefly in text and illustrated, including cardinality, in an appendix.
Queries: Three questions for the database are provided.
Conclusion: Conclusion includes a recap of the paper and insights from completing the assignment.
