1. Using the CARER protocol and/or LINDOCARRF standard approach to primary patient care, list all questions that you would ask this patient before making an assessment and recommendation.
NOTE: Only the C (Check) and A (Assess) from CARER should be used. This is because you don’t have any responses from your patient, so you can’t “Respond, Explain or Record”. LINDOCARRF may not be needed for all patient scenarios.
2. Provide a justification for each question you intend to ask the patient. You must explain:
the purpose of asking each question
what type of information you hope to gain from the question, in relation to the patient’s understanding of the illness and needs
how the answer to a specific question might influence your assessment and recommendation
3. Format
One (1) page written response: standard margins (2.5 cm), font: Times New Roman, font size: 12 points, single space. Use the template provided below.
A list of references used can be included on a second page to justify the questions you ask
