Proposed Initiative and Overview of the Organization
Homebound patients experience a variety of different medical problems. Mobility, social participation, and independence may all be restricted as a result of these conditions. Patients may experience an inability to leave the home, hence requiring home visits from the medical team. As a result, patients experience feelings of loneliness, isolation, helplessness, and hopelessness. These feelings often lead to suicidal thoughts or in some cases, completion of suicide.
According to Centers for Medicare and Medicaid (2022), A Home Health Agency (HHA) is an agency or organization which is primarily engaged in providing skilled nursing services and other therapeutic services. Home health care is provided to patients by nurses, therapists and other medical personnel. Once a patient is admitted to this setting, a depression screening is done at admission and will be done every 90 days for recertification to determine validation of continued services.
My proposed initiative addresses the need to screen the patients more frequently. This initiative requires patients be screened for suicidal thoughts/risk every week during the nursing visit instead of at recertification in order to refer patients to a behavioral health treatment team in a timely manner.

Strategic Issue: Improving Education
Goal: To improve the knowledge and assessment skills of healthcare service providers. There will be periodic continuing education about mental health and suicidal risk assessment. A combination of these strategies will ensure the professionals get additional skills and knowledge that will enable them to deliver better services and referrals.
Strategic Issue: Conduct Research
Goal: To understand depressive symptoms, mental health problems and discover better ways to treat them.
Strategies: Suicide is a major public health problem and a cause of premature mortality. With a view to prevention, a great deal of research has been devoted to the determinants of suicide, focusing mostly on individual risk factors, particularly depression (Tegeler et al., 2020).
Studies will be conducted on mental health diagnosis and safety issues. Research will be carried out to make sure common issues with homebound patients are understood and this will make it easier to deal with the problems.
Strategic Issue: Improve patient self-awareness.
Goal: To teach relaxation techniques and ways to reduce stress, teach patients positive self-affirmation.
Strategies: The organization will achieve this by encouraging professionals to be creative and innovative. Videos, exercises and apps that teach these techniques will be provided to nurses. This will enable them to come up with better ways to provide healthcare services to patients. Efficiency and effectiveness are essential as far as strategic improvements are concerned.
Operationalizing

Execution
The achievement of the strategic plan requires input from different stakeholders and relevant resources should be provided in the right quantities. For every tactic in the strategic plan, there will be an internal memo that will be addressed to all stakeholders. Communication is a key issue in strategic planning as it directly influences the chances of success (Shneyder, 2022). Stakeholders will also be equipped with resources and any other tools and equipment that they need to get the work done. There will also be motivation in different ways, such as allowances and bonuses. These are important incentives as they will encourage the stakeholders to take part in the process.
The chief operating officer of the organization will be accountable for ensuring that everything is executed according to plan. To achieve this, he needs the support of other stakeholders that will head different parts of the strategic plan. For a strategic plan to be successful, each stakeholder needs to play their role effectively. One of the most important stakeholders, in this case, is the human resources manager. This officer will make sure that proper education is provided to employees so that they are equipped with the relevant skills and knowledge required in the organization. The research and development officer will be tasked with the responsibility of ensuring that all research processes are carried out effectively. They will give an account of the research process and ensure that resources are also used effectively. Lastly, supervisors will oversee all operations that are intended to improve the healthcare of homebound patients.
Monitoring
Monitoring is an important aspect of strategic planning as it helps stakeholders to keep track of their performance (Martin, 2018). It also enables the management team to understand the progress of the plan. One of the key performance indicators that will be used to monitor is the skills and knowledge of the nurses. With increased education, it is expected that they will be more knowledgeable. The discovery of new and better strategies to assess homebound patients will indicate that the strategic plan is headed in the right direction. Lastly, an increase in the quality of services will be an overall monitoring tool. This will be determined by the level of satisfaction of the patients.
