There are many determinants of health that influence our health and its outcome. One of these is social environment, according to Olden (2019), “The social environment includes factors such as socioeconomic status, availability of fresh food, job opportunities, social interaction and support, discrimination, education, language, poverty, prevailing attitudes, and neighbors” (p.5). Olden goes on to say that “a person’s zip code is a stronger predictor of health than is his genetic code” (2019, p.5). Social determinants of health can cause health disparities based on where a person lives. Someone that lives in a rural area will not have the same access to hospitals and preventative care as someone that lives in an urban area. In a rural area there will be a limited number of practitioners that you are able to see. Some towns might only have one or two primary care doctors and no hospitals nearby. The quality might also be lacking if that one doctor is not really a good one. In a bigger city, that same person would have many options and if a doctor was not able to help, they could always choose to go to another one. There would also be more options when it comes to medical specialties. A family practitioner might not be able to help someone with certain neurological issues, they would have to see a specialist for that. This makes it harder for people to get the proper health care and could mean a missed diagnosis.
According to Lategan et. al, “A medical practitioner’s primary concern is the wellbeing of the patient: the best interests of the patient should always bear more weight than any other consideration” (2017, p.69). Sometimes determinants of health and health disparities can affect the judgement of a medical practitioner. Someone that comes to the emergency room with uncontrolled diabetes and is in bad condition could be judged and not taken as seriously. The medical team might think that this person does not care about their health and just lets themself go. This could cause inferior medical treatment due to assumptions, when in reality, the patient might not have been properly educated on the subject. This person could also lack health insurance so they cannot afford to go to the doctor or get the correct medication. Unfortunately, sometimes the interests of the patient do not bear more weight. We could go as far as to argue that the fact that health disparities exist is a big part of the ethical issues within the U.S healthcare industry. Lack of healthcare for all and disparities based on sex and race are all ethical issues that are present in the US healthcare system today.
The US is a very diverse country, this is a great advantage, and it helps us in the medical field because “diversity also contributes to a broad range of thoughts and attitudes about health and healthcare” (Wagner, 2021, p.83). Being diverse gives us better understanding of people and better cultural competency in healthcare, “Cultural competence underscores the ability to meet others where they are and effectively engage with people of diverse backgrounds.” (Wagner, 2021, p83). Wagner tells us that “If providers look at the world only through their own limited cultural perspective, the delivery of services may suffer, and access for patients from other cultures may be compromised.” (2021 p.84). Cultural competence as a healthcare manager means that you can understand your team better. The DC, VA and MD (DMV) area is very diverse and there are other areas in the US that are very diverse as well. In this area there are people from many backgrounds that are in the healthcare field. This sometimes can lead to lack of understanding or communication because of how cultures can be so different. Something that can sound rude to a person, can be completely normal to others depending on their culture. Instead of reprimanding that employee that sounded rude or seemed defiant, you can take them aside and try to understand their point of view. After this you might realize that the person was not coming from a bad place. If you can understand that as a manager, you can take a better approach to solving problems that are happening in your workplace.
