Mrs. J is a 53-year old black female who seeks follow-up at a veteran’s clinic. She has a long history of bipolar disorder and has been stable on lithium and olanzapine. She states that in the last several years her weight has increased from 155 lbs to 217 lbs. She is 5 feet 4 inches. She complaints of frequent urination, thirst and blurred vision. FBS is 210 mg/dl, Hgb A1c is 9.4%, BUN is 83 mg/dl, Cr 3.4 mg/dl, Pth 83 pg/ml, TSH 17, cholesterol is 230 mg/dl, LDL 163 g/dl, bp 164/93.
