Mandy Dawson is a 4-month-old infant who has been in good health since birth. Her mother brings her to the Well Baby Clinic for her routine check-up. She weighed 6 lb, 2oz at birth and is being breastfed. Her father is a manager of a retail store and her mother works outside the home. Mandy and her 3-year-old brother, Thomas, go to a local daycare for children ages 6 weeks old to 4 years. At the conclusion of Mandy’s 2-month check-up, her mother asked if anything could be done to prevent the discomfort of Mandy’s immunizations. Mandy’s pediatrician explains that she does not premedicate infants before immunizations although she would be willing to use ethyl chloride spray prior to the injection. Further, she states she doesn’t feel it is necessary because Mandy will not remember the brief discomfort of the immunization.
Case Study
Mandy weighs 10 lbs, 4oz at this visit and appears healthy. Her mother says she is smiling and plays with her feet, often putting her foot in her mouth. She turns from her back to her abdomen and can bear weight when being held in a standing position. She reaches for objects using her palm when attempting to grasp an object.
When the nurse prepares Mandy’s routine 4-month immunizations, Mandy’s mother (Mrs. Dawson) states she cannot stand to see Mandy hurt and has decided against having her receive any more “shots” until something can be done so that Mandy doesn’t experience pain from the shots. She further states that she is not going to bring Thomas in for any further immunizations either because he cries as soon as he enters the pediatrician’s office and is inconsolable when he sees the nurse.
