Ethical Principles Case Study
You are a healthcare administrator of a medium size long term care in Ontario. Ruth is an 82-year-old woman living this long-term care home. She moved in about 5 years ago when she started showing early signs and symptoms of dementia. At that time, she informed the staff that she has a son who’s estranged from her, and they have had no contact with each other for several years. She did not appoint a power of attorney for herself either. During the years, Ruth made her own decisions about all aspects of her life and treatments however over the years staff noticed a severe decline in Ruth’s cognition and physical health and started to question if Ruth is making the right decisions for herself.
Below are some of the challenges which require decision making:
Ruth likes to spend a lot of money on junk food, magazines, and gifts for her co-residents however her income from all sources is not sufficient to pay for her accommodation fees.
When Ruth moved in, she decided that if she gets ill, she should get full medical attention, up to and including CPR, transfer to hospital and ICU. However, now staff question if she can tolerate CPR and its consequences and given her fragile health, if multiple hospital admissions are in her best interest.
Ruth has developed a swallowing difficulty which is worsening; however, she refuses to eat foods that are minced or pureed which pose lower risk for choking. Staff do not know what to do since she may choke when she receives regular foods or starve since she refuses minced or pureed foods
If Ruth’s swallowing problem progresses, she may need to receive tube feeding. Ruth has not identified if she would like to receive tube feeding (if necessary) to sustain her life and her doctor thinks it may be necessary, in the near future.
Ruth refuses to take her medications at times which causes her to have inappropriate behaviours. Staff think they can hide her medications in her food.
