History

MK is a 65-year-old African American male who reported to the client for continuous management of a major depressive disorder diagnosed two months ago. MK reported that he had been taking PO Zoloft 50 mg daily. He, however, reports that the drug has not been giving him the desired outcomes. He still feels depressed with symptoms such as sadness, anger, poor concentration, a loss of interest in daily activities, and suicidal thoughts. MK needed to be treated again to embark on his job.

Objective

MK was poorly groomed, with huge pants and an oversized creased shirt. He was well-oriented to time, place, and person. During the interview, he looked worried and sad, and his concentration and attention were poor. No delusions or hallucinations were noted.

Plan

The therapist replaced PO Zoloft 50 mg with PO Prozac 20 mg daily for four weeks. The patient was counseled on drug compliance’s importance and to report any unusual symptoms. Cognitive-behavioral therapy was arranged in the following weeks. Further follow-up was arranged in 4 weeks.
