A 66-year-old left-handed female who lives alone is discharged from the hospital three days after a right, modified radical mastectomy for breast cancer. She has residual weakness of her left arm. due to H10 stroke and is unable to care for her breast wound. Her medications include Tamoxifen for breast cancer chemotherapy and pain medications. Her physician reports that the patient’s breast cancer is not resolved and the surgical drain is scheduled to be removed in several days. Skilled Nursing Need: The patient’s physician ordered skilled nursing visits daily for 10 days until the surgical drain is removed. then three times a week for four weeks to provide surgical wound care and supervision of the exercises ordered to improve her right shoulder range of motion and to monitor for any signs of infection. Skilled Therapy Need: Skilled therapy services are not required or ordered by the patient’s physician.
