After the hospital stay, the patient was transferred to an acute rehabilitation facility and he improved dramatically over the past four weeks. He was discharged from the rehabilitation facility two days ago and will begin outpatient physical therapy tomorrow. During this office visit his fracture was evaluated. The physician also reviewed and signed the physical therapy plan of treatment and order for the patient to continue to receive fracture aftercare.
