Rectal bleeding

The 86 year old woman was admitted with rectal bleeding.  She was also massively dehydrated, with a BUN of 124.  On admission, some IV fluids and transfusions of whole blood via a peripheral vein were administered because her initial hemoglobin was 9.5 and later dropped to 7.4.  On colonoscopy, multiple ulcers of the rectum, consistent with ulcerative proctitis, were found and biopsies were taken.  The tissue was negative for neoplastic disease and the patient was started on steroid enemas, with resolution.

Discharge diagnoses:  1) rectal bleeding, 2) dehydration, 3) acute blood loss anemia, 4) ulcerative proctitis
