Paul is continent. His verbal communication skills are almost non-existent. He is now working in a sheltered workshop environment. Paul has only been in this environment for two days.
Day 1
Paul threw a hammer, narrowly missing one of the instructors. He also dropped a wood working saw on his foot, sustaining a mild injury.
Day 2
The workshop instructors are very concerned about Paul’s self-harm practices. He is pulling his hair out, (little left in place now), and slapping his face, sustaining mild bruising and a skin tear from his finger nails. His finger nails have now been cut to a safe length.
