A significant issue that plagues the healthcare system in the United States is the high rate of hospital readmissions for patients with congestive heart failure (CHF). The 30-day readmission rate for CHF was 21.8 percent in 2012, significantly higher than that for other conditions such as acute myocardial infarction or pneumonia.
The problem statement for my HAC is as follows:
The high rate of hospital readmissions for patients with congestive heart failure (CHF) is a significant problem in the United States. In 2012, the 30-day readmission rate for CHF was 21.8 percent, which was much higher than the rates for other conditions such as acute myocardial infarction (AMI) (9.6 percent) and pneumonia (11.1 percent) (Wang et al., 2016). This high rate of readmissions is costly, both in terms of money and the impact on patient’s quality of life. In addition, the high rate of readmissions for CHF is a significant burden on the healthcare system.
There are several reasons why the readmission rate for CHF is so high. One reason is that CHF is a chronic condition, and patients often have multiple comorbidities. This means they are more likely to be readmitted to the hospital after discharge because their situation is more complex and more likely to develop complications. In addition, CHF patients are often older and frailer and may not have strong social support networks to help them manage their condition. Finally, the treatments for CHF are often complex and require close monitoring, which can be difficult for patients to do on their own.

Mixed Methods Checklist
Does the purpose statement fit logically with the problem statement?
Does the purpose statement use similar words to the problem statement, or does it seem to address a different topic?
Based on the problem statement, do the research questions align with the method and design of the study? For instance, are words like perception used that would automatically reject quantitative research?
Do questions seek to describe responses to the variables described in the study?
Do research questions appear to fit the criteria for qualitative and quantitative research?
Are hypotheses associated with the quantitative research questions?
Does the researcher indicate how the two approaches will be integrated into the study?
Does the question specify the participants and research site? Does the description describe the overall intent of the study to justify the use of a mixed method?
