For Large Group Assignment Discussion:
Important contextual, demographic, or descriptive information about this client.
Presenting problem (what do you see as a problem that might bring the client to seek help?)
How would the client describe the problem (or problems)?
How long has this problem, or these problems, been around?
Describe behaviors that are attempts to cope with the problem (e.g., substance use, avoidance, blaming others, etc.)

Using the DSM-5 diagnostic system, state your diagnosis. Be sure to include the diagnostic code, the proper name for the disorder, and any relevant specifiers. What diagnosis would you give this client? Why?
What diagnostic assessment might you use to formally diagnose this disorder?
What criteria for the diagnosis are met?
What evidence do you have to support your diagnosis?
What type of treatment would be most appropriate for this client given their diagnosis?
Describe how the client’s gender, race, ethnicity, cultural background, sexual orientation, or other issues of diversity might affect his or her diagnosis and treatment plan?
What historical, social, political, cultural, or religious issues do you need to consider in applying this diagnosis?
What ethical issues arise in applying this diagnosis (e.g., social stigma, labeling, etc.).
What are the client’s strengths and how can they be utilized in the counseling process/treatment plan?
Identify Long and Short Term Goals: The goals should be linked to the diagnosis and the needs of the client.
What are short term goals for this client?
What are long term goals for this client?
Specific Considerations for Treatment:
What is the anticipated length of treatment?
Is the client motivated to seek treatment?
If not, how do we encourage motivation?
Describe any previous treatment attempts.
Assess the client’s social support system.
How might the severity or nature of disorder affect treatment?
How might cultural variables affect treatment?
What additional services (besides counseling) might be beneficial for this client? (Medications, in-patient services, other groups, additional therapies, etc.)
