- How would your communication and interview techniques for building a health history differ with each patient?
- How might you target your questions for building a health history based on the patient’s social determinants of health?
- What risk assessment instruments would be appropriate to use with each patient, or what questions would you ask each patient to assess his or her health risks?
- Identify any potential health-related risks based upon the patient’s age, gender, ethnicity, or environmental setting that should be taken into consideration.
- Select one of the risk assessment instruments presented in Chapter 1 or Chapter 5 of the Seidel's Guide to Physical Examination text, or another tool with which you are familiar, related to your selected patient.
- Develop at least five targeted questions you would ask your selected patient to assess his or her health risks and begin building a health history.

Class,
As we make the transition from an RN to an NP, we have to start thinking and speaking like a practicing NP.
Many of you are struggling with your references and may have some confusion as to what constitutes an evidenced base reference/resource.
Why do we care about Evidence Based Resources?
These sources utilize the most up to date research on how diseases occur and are diagnosed as well as the newest and most effective treatment guidelines. In layman's terms if you don't use an EVB when designing a treatment plan you may be choosing a treatment that has been proven ineffective or just inappropriate for the situation. In other words, you can wind up doing the wrong thing to and for your patient :(. And you want to deliver the best care to your patients. Just as a patient yourself, you do not want your provider giving you a medication that isn't the best choice for your disorder or doing a procedure incorrectly that might harm you or delay treatment. How do I Know Which Resources are EVB? Resources are EVB when they are: -Peer Reviewed journal articles that reflect and discuss the results of a research study Examples of Peer-Reviewed Journals include: -American Family Physician -American Academy of Family Practice -American Thoracic Society -National Heart/Lung and Blood Institute (Clinicians tab, not patient tab) -American Academy of Dermatology…… Is the Article Speaking to ME, the clinician, or Staff Nurses, or Patients? The last issue I find students encounter is using an article that isn't speaking to clinicians and therefore does not demonstrate evidence and research geared at treating patients. This is where the NHLBI can be a tricky site. Many articles are meant to educate patients and so the information is watered down for their understanding. These articles are appropriate for use in your patient education but not as rationale for your diagnosis or treatment. Two examples are provided below: Example one: An article for Lay people and not evidence based research: National Institute of Alcohol Abuse and Alcoholism. (2015). Alcohol’s effect on the body. Retrieved from: http://www.niaaa.nih.gov/alcohol-health/alcohols-effects-body Links to an external site. This article begins with "This is how you know if you have an alcohol problem….." So the YOU is the patient and the article is speaking to the Patient and not the Provider…… Journals that speak to staff nurses and healthcare professionals other than NP, PA, MD, Surgeons etc…also do not provide the best evidence and are not directed and diagnosing and treating disease. Examples of these journals include: -Practice Nurse -Critical Care Nurse We will see how the use of Evidence-Based resources impacts your ability to choose the correct treatments and interventions for patients as we go through the course. Our goal is to make you excellent providers who deliver excellent care! When submitting scholarly work to (or any other institution of Higher Education) for approval or a grade you must follow the institution’s rules for submission. If the Nurse Practitioner Program does not recognize a reference source or deems it as inadmissible, you should not use it. As advanced practice nurses, we are scholars, nurse researchers and scientist, and as such should only use Peer-Reviewed scholarly articles and websites designed for health professionals (not designed for patients) to reference scholarly work. We should also use these scholarly works as a basis for our patient education and recommendations to patients in the clinical setting.
The Nurse Practitioner Program does not deem websites designed for patients or written in “layman’s terms” as admissible for scholarly work in any course assignment. Even some resources written for professionals, such as Up to Date are not admissible as a scholarly reference. Students should be using the original citation in Up to Date and go to that literature as a reference. The following are examples (not all inclusive) of resources/websites deemed inadmissible for scholarly reference:
1 Up to Date (must use original articles from Up to Date as a resource)
2. Wikipedia
3. Cdc.gov- non healthcare professionals section
4. Webmd.com (This will make Dr. Schiller crazy!)
5. Mayoclinic.com
6. Babyzone.com
7. americanheartassociation.comLinks to an external site.
