The determination of the appropriate intervention, given the assessment and diagnosis of community issues, is critical. The PHN works with the community stakeholders to select an intervention specifically targeted at one of the issues identified.

Please complete the following steps for your discussion post and response:

Choose a different intervention that you could use for the community health issue identified in your project. This is not the intervention you have chosen for your project but a new way to approach the identified issue.
Describe the issue and the intervention you would implement in response to it.
Discuss why that would be an appropriate intervention for that issue.
Include the level of prevention, steps required to complete the intervention, and how you would evaluate it.
