You are in charge of the quality improvement team at your home health agency. Your team has identified 4 areas of potential improvement based on data collected from OASIS and OBQI reports: 1) falls, 2) new infections, 3) volunteer referrals, and 4) patient and family education. As a home health agency, you cannot mandate that the family does anything; you can only recommend and suggest. Make sure to include references for any materials you accessed. you will choose one of the 4 areas of concern and make a QI plan. Since you will be presenting this to your agency’s medical director, you need to make sure that your plan includes the following points: ● A brief description of the problem you are attempting to address ● The causes that are contributing to this problem ● An action plan to address this problem ● Markers you are using to measure the need for and success, and/or failure of this plan ● What your goals are for this QI plan ● How you will address positive and negative outcomes of your plan Remember, your plan must be realistic, and you must include how all members of the interdisciplinary team will contribute to the plan, including the RN, CNA, physical therapist, respiratory therapist, and the physician.
