Directions: The purpose of this assignment is to assist you in understanding how individuals change unhealthy behavior. Have your participant select one behavior to reduce/eliminate or one health-promoting behavior they wish to add/increase. You are going to examine and that behavior, using the following health behavior theories: Health Belief Model (HBM) and the Transtheoretical Model (TTM). Then you will prepare an intervention that aligns with the theory and utilizes the Multiple Intelligences (MI). make the response section for questions 1-6 clear.
1. Target participant:
Provide a brief background on the person you chose to work with and why. Identify their desired behavior to change and the reasons why they wanted to make this change.
2. Assessment using Theory:
prepare a Diagram of the Health Belief Model and identify each of the key constructs.
Apply your participant’s responses from the questionnaire to each of those constructs in the Health Belief Model.
3. Application of the Transtheoretical Model (TTM):
Using the TTM identify what stage of change your participant is presently in (at the start of this assignment). Identify the stage, and provide some key characteristics that would support your selection of that stage of change.
Next identify what stage you trying to assist them to move to next and why? How will you encourage them to move?
4. Application of a Behavior Change Intervention Plan:
prepare a brief summary statement of your overall intervention (plan) that you would create to assist the person to engage in.
o This should be a realistic plan that the participant can do.
o Identify how you will use information collected from the Theories to help shape your plan.
5. Integrating Multiple Intelligences
Identify how you would use three of Gardner’s Multiple Intelligence (MI) to encourage your participant to change their behavior.
o Think about what your participant would benefit from.
o Provide examples of how you would use the Intelligence in your intervention for the participant to be successful.
6. Self-Reflective Statement:
Provide a three paragraph reflection on what you have learned and how it will be helpful in your future as a health educator in your given worksite.
