Tell us how your loyalty to the clinic was shaken and why.  Did they try to resolve the issue or did they lose a patient for life?  You may be as specific as you are comfortable with, but you must not give out any medical information about yourself or loved ones. (Diagnoses, etc.)  An example might be a time the office gave you the wrong medication. Notice I didn’t say what meds nor what it was for, but we can all agree that giving a patient the wrong meds is a huge error.  Perhaps they lost your labs; again non-specific to disease, but equally bad.  If this assignment is a trigger for you, like something involving a traumatic experience that you do not want to re-live, .
