Read the following three questions carefully before answering. These questions need to reflect three (3) different situations you have had with a person you support. NOTE: If you are unable to reflect on three (3) different situations with one (1) person, you can use two (2) people. Give a brief description/understanding of each situation before addressing the questions.
Describe how you became aware and identified the potential unmet need/s with the person you support and what they were. Consider the following points in your response:
What were the signs you recognised and how did they show themselves?
What information did you need or methods did you use to determine further investigation, fully assess and confirm the scope and nature of the unmet needs?
Other people that were involved or should have been involved in the assessment process and the role they had.
What information is collected and reported, analysed and developed with individualised plans for the person?
Describe the strategies developed to meet and support the person’s needs. Consider the following points:
What role did you have in the development of the strategies and how did the they respected the persons dignity, rights and personal choices?
What approaches(if any), were incorporated to address impaired social judgement and decision-making capacity including any limits and boundaries set?
The integration of individual active, reactive and crisis responses used based on individual needs that reflected and where in accordance with duty of care, ethical and legal requirements.
The colleagues and stakeholders consulted and played an integral part of the planning process and how this was formally documented in the persons individualised plan.
From the changes developed and made to the person individualised plan, describe how effective they were and how they were monitored. Consider the following points in your responses:
How the strategies were monitored and were able to maintain positive and adaptive responses?
How effectively monitoring the strategies enabled the reduction of, risk of harm to the person and others working with them, the persons dignity and self-esteem?
The review and recommended modifications of individualised plans, the staff and stakeholders consulted in the process and the reports and other documentation completed to ensure ongoing currency.
