LOCATION: Hospital Emergency Department PATIENT: Taylen Long PHYSICIAN: Paul Sutton, M.D. SUBJECTIVE: This is a 9-year-old male who has no significant ongoing medical problems, takes no medications, and has no known allergies. He was seen last evening in the emergency room by myself with abdominal discomfort and was felt to have a possible viral illness and was discharged to home for observation. He returns this evening with complaints of abdominal discomfort pointing to the epigastrium, which seems to be worse to him than it was last night. There is some generalized periumbilical tenderness discomfort as well. He continues to have no nausea, no vomiting, and no diarrhea. He has no urinary tract symptoms. His appetite has been okay today. He has been eating normally. REVIEW OF SYSTEMS: Constitutional: No fever. No sweats or chills. GI: As outlined above. Otherwise negative. GU: No urgency, frequency, or dysuria. Respiratory: No cough. No shortness of breath.
