Reflect on a patient that you assessed during your clinical experience this week or last week.
Instructions:
Select one of the tools/instruments presented in Unit 2 Week 4, or one you locate on your own, and construct a discussion that encompasses your assessment findings relative to your patient’s social determinants of health.
Reconstruct the elements of your patient’s ‘Plan’ that encompasses interventions aimed at challenges/issues identified for one of your patient’s social determinants of health categories.
Include citations for each of the resources you used to support the elements of your Plan.
Responses need to address all components of the question, demonstrate critical thinking and analysis and include peer-reviewed journal evidence to support the student’s position.
Please be sure to validate your opinions and ideas with in-text citations and corresponding references in APA format.
Please review the rubric to ensure that your response meets the criteria.
Please respond to at least 2 of your peer’s posts with substantive comments using the following steps:
Construct a response to your peers commenting on their Plan elements that relate to the identified social determinant of health.
For each colleague that you respond to recommend an additional intervention to address the identified social determinant of health and support the intervention, you add to the plan with a scholarly resource.
Substantive comments add to the discussion and provide your fellow students with information that will enhance the learning environment.
References and citations should conform to APA standards.
