Writing a Population Diagnosis
Based upon the concepts learned during Week 1 and your work completed in NR 538, respond to the following:
Identify at least two population needs and two population problems based on your population or community assessment data and analysis completed in NR 538.
Your selection of a population or community diagnosis establishes the priority focus to move forward with the lessons in this course as well as for your Population Health Practicum (CGE- Concluding Graduate Experience) and Project. After analyzing and synthesizing your population or community assessment data from NR 538, formulate a population diagnosis. Use the Community-As-Partner format to write your diagnosis. (See Ervin & Kulbok (2018) – Chapter 8 or Anderson & McFarlane (2019) – Chapter 12 for the structure and formatting of diagnoses using the Community-As-Partner format.)

Consider your learning throughout week two regarding frameworks to guide program planning and your plans for the Concluding Graduate Experience (CGE) and building a population-focused program. Respond to the following:
Discuss your chosen priority health risk or disparity and target population and provide your initial thoughts on creating a program/project to address this population issue with your Practicum and Project.
What population-focused health promotion and disease prevention activities or programs are already in place for your population or community related to your health risk or disparity focus area?
Identify and discuss a program planning framework or model from Ervin & Kulbok (2018) that will be useful to guide your program planning process.

Developing a Population Health Program Plan to Address Human Trafficking
Based upon this week’s readings, read the selected article [Guidance on spotting possible victims of human trafficking (2018)] and the pamphlet on human trafficking [Human trafficking: The basics of what nurses need to know (2019)]. Visit the online National Human Trafficking Resource Center.
Respond to the following:
Select a healthcare setting of your choice (hospital, ER, primary care clinic, community center clinic, local public health department clinic, etc.) that human trafficking victims may present for healthcare. Brainstorm on ideas for a program plan to help human trafficking victims in the selected healthcare setting.
Who are the stakeholders to include in the planning process that may serve on a planning committee?
Discuss interprofessional collaboration that will be needed for program planning.
Identify at least two ethical and two legal considerations to keep in mind with the program planning process.

Based upon your plans for your program and target population, let’s continue the discussions with the class building on the Week 2 Collaboration Café and your work completed thus far on your program planning and implementation efforts. Respond to the following:

Discuss a possible practicum agency for the population of focus.
What program implementation challenges might you encounter?
Who are the stakeholders to include and consider as you are developing the implementation strategies?

Program Evaluation for a Target Population
Now that you’ve created program implementation strategies for your target population, consider the appropriate evaluation strategies and respond to the following:
What are the purposes of conducting a program evaluation with your target population and program implementation plans?
What is the relationship of program evaluation to your population assessment, diagnosis, goals, objectives, program planning, and program implementation strategies?
Select and discuss an evaluation model or framework that may be helpful for guiding your evaluation planning process.
What roles do interprofessional team members and population members and/or stakeholders play in the program evaluation process?

Week 6 Discussion
Collaboration Cafe: Population Health Program Evaluation Methods
Identify a population-based program evaluation in place at a clinical site (could be a hospital setting, primary care clinic, community center clinic, local public health department clinic, etc.), This site could also be your current workplace. Select a different site than the one you plan on using for the Concluding Graduate Experience (CGE) or population-focused program development that you’ve been spending time on related to your target population. Select a program that has a different target population than the one you’ve been exploring. Conduct a brief interview of a key stakeholder for the program to assist with responding to the following:
What population are they addressing with the program?
What population health issue is the program addressing?
What methods are in place for their evaluation process of a program?
Are they effectively measuring their program outcomes with the evaluation process?

Week 7 Discussion
Sustainability of a Population-based Program
Reflect and analyze your work completed thus far with the population health program planning, developing implementation strategies, and an evaluation plan for your target population.
Respond to the following:
Brainstorm on ideas for how you could sustain your established program and share with the class.
Are there internal or external threats to the sustainability of the program?
Identify and discuss at least two evidence-based websites that support population- based nursing and that may be helpful for examining your evaluation data, exploring program sustainability, or strategizing on program revisions if needed.
