I chose Cognitive Behavioral Therapy and its application in Ella’s case study. CBT focuses on identifying the root causes of harmful behavior. This therapeutic approach aims to identify the various biases brought about by these unhelpful methods of thinking and find ways to deal with the issues the patient is facing (Stallard, 2022).

Habigzang et al. (2018) did a study to evaluate CBT’s impact on survivors of domestic violence in Brazil. As one of the ways to assist these survivors, the researchers utilized CBT to help these women, who were also dealing with psychiatric disorders such as PTSD, anxiety disorders, and substance abuse as a result of domestic abuse. CBT was used to negate the impacts of the violence. The researchers utilized the Mindfulness-Based Stress Reduction (MBSR) technique to assess the effects of CBT on the sample to determine the effectiveness of the treatment approach (Habigzang et al., 2018). They performed a pre-test evaluation, which was followed by the use of CBT, and then a post-test evaluation on 11 participants. The study initially had 120 participants, but several participants were ineligible for the study due to inconsistent attending sessions and cognitive hindrances. The study identified that CBT was very effective in helping the clients to deal with the psychological effects of the abuse (Habigzang et al., 2018).

Cognitive Behavioral Therapy can be beneficial to Ella. It can help Ella deal with the post-traumatic stress disorder she may be dealing with due to the verbal, psychological, and physical abuse she has dealt with in the past (Stallard, 2022). According to the report, she has begun exhibiting signs of acute distress and trauma, which could be signs of PTSD. CBT can be beneficial in helping Ella deal with this PTSD and helping her learn to cope with the past traumas she has experienced.
Determine whether you would use or not use the therapy you selected for the client in your selected case study (consider how culturally relevant it is, how aligned it is with social work ethics, etc.) and explain why.
While working with Ella, I would use cognitive behavioral therapy as one of the treatment approaches. This is because CBT can be very effective in helping Ella deal with the various effects of the trauma she experienced before she ran away from home. Only after dealing with this trauma can she take steps to fix herself and figure herself out. This can serve as a foundation for her to begin reuniting with society, which includes having a relationship with her mother.

Adolescents completed self-report questionnaires to provide the following information: depressive symptoms, experiential avoidance, as well as some physical health information. Based on their reported depressive symptom and avoidance scores, adolescents were split into three groups and provided each with a 5-week online ACT intervention. Adolescents with the most severe symptoms appeared to benefit from ACT treatment, adolescents with average levels of these symptoms did not see a decrease with the implementation of ACT, and adolescents at low levels of symptoms saw a decrease when treatment was implemented (Keinonen, et. al., 2021).
Explain how the findings are applicable or appropriate for the client in your case study.
The researchers learned about the effectiveness of ACT when provided online to adolescents, which Ella is. She also may be experiencing depression or experiential avoidance.
Determine whether you would or would not use the therapy you selected for the client in your selected case study and explain why.
Due to the fact that this study shows effectiveness in adolescents with high and low levels of symptoms, but not in adolescents with moderate symptoms (Keinonen, et. al., 2021), I am not yet sure if I would apply this therapy for her intervention. I would first want to give her a chance to fill out the self-assessments used in this study to help determine if ACT would be beneficial to her.

The research method for exploring the effectiveness of adjuvant interventions that lead to more symptom reduction in adult PTSD patients receiving TF-CBT was literature and systematic review. The data was collected electronically by searching databases for controlled clinical trials. Of these, thirteen trials fit the inclusion criteria and were evaluated for internal risk of bias using the Cochrane Handbook for Systematic Review of Interventions (Schanz et al., 2019).

Sam is a combat veteran, and because of this event, he is suffering from PTSD, anxiety, and depression. He turned to alcohol to cope with what he had experienced during his deployment. TF-CBT will help when discussing the traumatic event, assist in the gradual learning to control his cravings, and learn a coping skill that will enable him to talk about what happened and his feelings without his anxiety levels and negative emotions increase.

Considering the effectiveness of cognitive behavioral therapy specifically focused on trauma, I would utilize this therapy. Turner (2017) explains that behavior is taught and can be both defined and changed through significant others in the person's environment or environmental transactions. In considering cultural relevancy, this therapy is adequate because it focuses on behaviors and education of behaviors. However, the social worker must know what the client's culture constitutes as unacceptable behaviors and which coping mechanisms are acceptable and appropriate. CBT aligns with the person's ethical standards of dignity and worth by seeking to enhance clients' capacity and opportunity to change and address their own needs (NASW, 2021).
