Hazard identification is part of the process used to evaluate if any situation, item, thing, etc. may have the potential to cause harm aka “What can go wrong?”. A hazard is any source of potential damage, harm or adverse health effects on something or someone (for example, to people as health effects, to organizations as property or equipment losses, or to the environment).
This week’s assignment has two parts: complete a Job Hazard Analysis for a task or process at your team’s chosen workplace and construct a risk matrix for an identified hazard.
Instructions:
Part A (60 points):
1. Using the provided format in Week 4 Reading, follow the steps to complete a Job Hazard Analysis for a task or process at your team’s chosen workplace.

c. Identify the hazards and hazard scenarios for each step.
d. Identify the control measures or prevention measures for each step.
e. Identify the person responsible for implementing the control or prevention measures.
Part B (40 points): Construct a 4 x 4 risk matrix to use to assess the risk associated with Part A of this assignment..
a. Design the probability and severity according to category. (20 points)
b. Calculate the level of the identified risk. (20 points)
