Ihuman Templates Fall 2022
There is a steep learning curve just to familiarize yourself with the platform, but once you understand and know how to navigate you will do well. You MUST look at the resources tips and videos they offer to help you navigate iHuman and DR Gatica’s PPX has a step by step outline. Open a split screen and follow along with the Marvin Webster tutorial case on your first attempt
You are limited to 60 questions in the interview and 40 exams. If you exceed this you will score zero. Once you complete a section you can not go back. Your submission is final, no re-do, re-open, re-send or erase. All 5 cases count 100 points each and 20% of your grade. You will complete the case in the human platform. All cases will be in learning mode. After you complete your ihuman you will post the pdf summary in the grade book ( watch r Gatica’s ppx). I will only grade your HPI and your Plan. The HPI and plan MUST be in iHuman. It it is not there you score zero (watch Dr Gatica’s PPX). The questions, exam, dx and test questions will be graded in ihuman by ihuman. I will use a grading scale to grade the rest. A copy of the grading scale is pending. Students that are experiencing technical issues should submit a request using the link on their i-Human homepage to ensure we get the information to assist them.

Be sure and write the HPI in the EHR. If it is not written in EHR I cannot see it or grade it. You will score zero, No excuse. Look at Dr Gatica’s PPX for step by step instructions

56 y/o AAF obese kinder garden teacher who has a hx of tobacco and COPD, presents with cough. States her cough started about 3 days ago and is progressively getting worse especially if her husband smokes. She states cough is mostly in her chest, loose and productive. Associated symptoms include, hoarse voice, brown sputum, and low grade fever or 100.3. She denies blood in her sputum, difficulty swallowing, or sinus drip. She feels her symptoms are worse at night, and with activity, especially outside. it helps to rest, and use her humidifier. She also tried her husbands inhaler which made her breath better. She is vaccinated and boosted for COVID, and her flu shot is UTD.
Risk factor: 2nd hand smoke, obesity, works in school, hx of 10px year tobacco- quit 6 mo ago
Fam HX: father died of lung cancer

EXAMPLE OF THE REQUIRED TEMPLATE FOR THE PLAN SECTION BASED ON THE MARVIN CASE (20 points)
1: Primary Diagnosis with ICD-10 code, rationale, and resources. (5 points)
Primary DX: Influenza, J10. 1 Classic signs of influenza are abrupt onset of fever, cough, myalgias, and fatigue which Marvin exhibited all of these symptoms (Johnson & Johnson, 2020). Furthermore, Marvin did not receive his current influenza vaccine, and his communal living situation (college dorm) placed him at higher risk for influenza (Smith, 2019). Furthermore, his RIDT test was positive.
DDX #1: COVID-19: Marvin’s respiratory symptoms and his higher temperature could potentially be COVID-19 (COVID, nd). Furthermore, the increase of COVID variants and Marvin’s lack of COVID booster make it necessary to consider this as a differential. This differential was ruled out with a negative PCR.
DDX #2: Bacterial URI: + RIDT. At risk because he lives in a school community. No fever or chills, cough with yellow/ brown sputum and lung sounds clear makes it less likely. (National institute of health. ND)
DDX #3. Asthma, exacerbation: R/o with exam that did not show any wheezing, no cobble stone nares or shiners around the eyes (https://www.aafp.org/pubs/afp/issues/2011/0701/p40.htm)
2. Medications including OTC, dosage, and education (5 pts )
Meds: Motrin 200mg q6 hours PRN for pain and fever
Symptoms tx: humidifier, gargling with salt and water
Education: Influenza is contagious. Wear a mask and wash hands frequently, council on red flags such as SOB. Return to clinic if hypoxic or febrile
3. Additional ancillary tests needed, referrals and follow up 5 pts

Follow up: RTC if sx worsen and for health maintenance
4. Social Determinants of Health/ Health Promotion (5 pts)
SDOH: Pt. has access to healthcare through student health. He has safe housing and support. He is educated and understand instructions
Health promotion: Advised pt. to have a yearly physical, yearly flu vaccine, and offer COVID booster
