Vulnerable Population Concept Map and Summary
Purpose
The purpose of this assignment is to analyze a vulnerable population within a selected community to identify health risks and disparities. Variables influencing the risk and disparity experienced by members of the vulnerable group will be identified, and correlation with national directives to address population health issues will be determined. A concept map will be created to portray the relationships between these elements. Implications for advanced nursing practice and opportunities to advocate for the health of the vulnerable population will be presented.

Course Outcomes
Through this assignment, the student will address the following course learning outcomes:
CO 3: Use epidemiological principles to determine population health risk and disparity (PO 5)
CO 4: Articulate advanced nursing practice roles and competencies to support population health (PO 3, 4)
Assignment Overview
Identify one vulnerable population within a selected community. Explore current literature and related data to better understand the variables that place this population at risk for health concerns and health disparities. Analyze national health directives (such as Healthy People or another national initiative) to determine correlation with needs/risks/disparities of the vulnerable population.
Part 1: Concept Map
Identify the following elements to include in the concept map.
Vulnerable population
Variables (risk factors) that place the identified population at risk (minimum of three)
Minimum of one health risk or disparity for which the population is at risk
2. Create a concept map?that portrays each element and the relationships between the vulnerable population, variables placing the population at risk, and the identified health risk or disparity for which the population is susceptible. The concept map must be your own, original work.
Assignment Instructions
Abide by the Chamberlain College of Nursing Academic Integrity Policy.
Part I: Concept Map—create and submit a concept map as a pdf or a Microsoft Word document.
Concept map may be either:
neatly drawn by hand and submitted as a pdf; or
created using Microsoft Word.
Concept map must be clear and legible.
Concept map must be the student’s original work.
Concept map must include the student’s name and date.
