Objective: Create an assessment report.

After conducting the screening process clinicians will determine the next course of action. This may mean determining if the client is eligible for services or not and if further assessments are needed. Once the intake and assessment is completed and all the data is gathered, the clinician will create an assessment report summarizing the client’s information, clinician’s observations, and necessary recommendations that will determine the course of treatment. For this assignment, you will practice typing an assessment report based upon last week’s case study.

• Type a 3-4 page assessment report based on the information from Week Three’s assignments (Case Study and Interview Process Response) and following the Global Criteria 12-16

To achieve maximum points for content and analysis, the following elements need to be thoroughly addressed.

• A summary of the case based on the history gathered.
• How the individual meets DSM-5 Dimensional Criteria for a diagnosis.
• Selection of assessment tools and the rationale for each.
• Brief description of the client’s strengths and weaknesses.
