Referral Information and Assessment Summary
Why did the family call for help?
Provide a summary of the key take away points from the indirect assessment
Summarize information from the interview and diagnostic summary
Provide a summary of the direct assessment
Briefly summarize your direct observation data (include the type of data collection forms that were utilized: A-B-C data, frequency, interval, duration, and latency)
Functional Analysis Assessment:
Review the provided functional analysis graph and identify the probable/hypothesized function
Explain the graph in understandable terms
Discuss your antecedent strategy based on the case scenario
- Discuss motivating operations/establishing operations, setting events, and discriminative stimuli as appropriate
Identify the research design that you will be using
- Explain your rationale for your choice
Behavior Intervention Plan
Antecedent Modifications
- Research Design
- Alternative/Replacement Behavior
What is your replacement behavior?
- How will you reinforce your behavior?
Reinforcement
What type of reinforcement contingency will you be using?
- What is your schedule of reinforcement (continuous or intermittent, fixed/variable, and ratio/interval)? Discuss your rationale.
- Did you conduct a preference assessment to identify a reinforcer?
What is your criteria for reinforcement?
Consequence Strategy
How will you react to the target behavior?
Discuss differential reinforcement (DR) and extinction if this is part of your BIP
- If using extinction, discuss how to prepare
Teaching
- How will you approach training? (Naturalistic training, discrete trial, contrived settings/events, etc.)
- Discuss your prompt hierarchy
Generalization and Maintenance
- Include your generalization and maintenance strategies here
Discuss how you will train others on the implementation of the BIP
- Discuss how you will collect data and what measurement system you will use
Add your criteria for mastery and discharge here
- How often will you work with Axel? What resources or supplies do you need?
Include your line graph with baseline and intervention here

Behavior Analyst Signature Date The pressure is on! You have been busy the last few weeks with direct observations of Axel at his home and community settings. You have operationally defined his target behaviors and chosen one to address with a behavior intervention plan (BIP). It is time to get creative!
Utilize the Unit 9 Assignment Template to write a behavior intervention plan for Axel addressing the target behavior you chose for the Unit 6 Assignment. You will include the following in the plan:
Name and date of birth of client.
Parents’ names and contact information referral Information.
- Referral information (why did the family call for help?) and summary of indirect assessment.
Briefly summarize the information you learned through interviews, records reviews, and questionnaires.
- Briefly summarize your direct observation data. Include the type of data collection forms that were utilized: A-B-C data; frequency; interval, duration, and latency.
After you completed A-B-C data, you completed a functional analysis. Review the graph below to confirm the probable/hypothesized function of the target behavior. Describe your findings.
Design a function-based behavior intervention plan:
Antecedent modifications: motivating operations/establishing operations; setting events, if appropriate.
Identify the research design you will be using and discuss your reasons for choosing the design (A-B design, reversal design, alternating treatments design, multiple baseline design, and changing criterion design).
Alternative/replacement behavior: How will you teach and reinforce this?
- Identify your reinforcement (positive; negative; non-contingent). Did you conduct a preference assessment?
- Schedule of reinforcement (continuous or intermittent). Discuss the reasons for your choices.
- Criteria for reinforcement.
- Differential reinforcement (DR) and extinction (if part of your BIP).
If using DR and extinction, for what effect(s) must you be prepared?
How will you approach training (naturalistic training; discrete trial; contrived settings/events, etc.)?
Will you use a prompt hierarchy? Briefly discuss.
Include generalization and maintenance approaches in your BIP.
- Discuss how you will train others in the implementation of the BIP using a behavioral skills training (BST) approach. Include a discussion on each of the steps used in BST. Include who will you train and how you will ensure that the plan will be implemented correctly.
- Data collection (for home, school, OT, and ABA therapy sessions). Continuous or interval data collection? What measurement system will you be using? Create a data sheet for collecting intervention data and add to the appendix of your behavior intervention plan.
Criteria for mastery and discharge that is related to your measurement system.
- How often will you be working one-on-one with Axel? How will you address the target behavior during therapy? What supplies/activities will you need?
