cristian post: There is plenty of evidence that health behavior such as smoking, substance abuse (drugs and alcohol), physical inactivity, and unhealthy eating are associated with increased morbidity and mortality. The challenge of addressing unhealthy lifestyle choice is complex and requires sustained behavior change. One approach to change motivation and subsequent behavior is Motivational Interviewing (MI) which is often recommended as an evidence-based approach. A definition of “MI is a collaborative, goal-oriented style of communication with particular attention to the language of change. It is designed to strengthen personal motivation for and commitment to a specific goal by eliciting and exploring the person’s own reasons for change within an atmosphere of acceptance and compassion.” (Miller & Rollnick, 2013, p. 29) The most notable is that MI is designed to empower people to change by drawing out their own meaning, importance and capacity for change. MI refrains from unsolicited advice, confronting, instructing, directing, or warning. Involving the patient’s family early in the process of change is crucial to success by having a support network at home. Engagement of the intact family is beneficial as a support system when you are in need, but also by providing for a family can serve as a potent source of motivation to make changes in health behaviors. The role of the family is to motivate, support, assist and encourage healthy habits and if it can be accomplished together is even more beneficial to all involved. Sometimes new healthy habits need to be supported. One can’t be eating healthy like a salad for lunch and their partner order a pizza at the same table. Supporting each other is also sharing the same beliefs and healthy habits.
Mohamed post: Education and communication are key factors in helping patients make better decision concerning their health. For example, this morning I had a patient who is a Jehova’s Witness refusing dialysis. When I asked the patient if they understood what dialysis entails and why they need it the patient showed no clear understanding. After explaining to them simply they were able to understand the procedure and make a more informative decision based on their beliefs. After some discussion with their family members the patient decided that dialysis would be in their best interest. Other effective ways to motivate patients in making good decisions in their health care is explaining the risks and benefits of the treatment plans recommended. When the patients have family you can use the members to also help motivate and make better decisions. Another example is a patient today who has grandkids was not following their insulin and accucheck protocol that was recommended to them and they were hospitalized for DKA. After explaining to them what the consequences maybe from not managing their diabetes they had a change of heart. Especially when told that they could lose their eyesight, limbs and life and could one day be unable to see or play with their grandchildren because of their non-compliance. These types of patients need continuous encouragement and reminders as to why it’s important to follow medical advise. I typically include family members in these discussions so that the patient is not as easily drawn back to those bad habits.
