Discharge Summaries are very important because they summarize the treatment needs and the course of treatment that met those needs in the client. They report the treatment progress of the client and their substance abuse status at the time they entered the treatment center. They report further treatment needed at termination of current treatment. It also, identifies aftercare and provides information on continuing care. The types of information to be included in a thorough discharge summary are the Initial presenting problem, diagnosis, treatment problems and goals, course of treatment, evaluation of the progress made, final assessment of needs, substance abuse status at discharge, referrals for further treatment, aftercare plans and final prognosis. The discharge summary should tell me, about why the client came in to seek our services, in what condition did the client came in, what was done about all the observations, assessments, tests, and evaluations, the course of treatment for the client, the progress he or she made during treatment through a particular time. It will also tell me the status at which the client is at the current moment and whether or not and/ or what treatments will be necessary for their aftercare and to prevent a relapse. Discharge summaries are a very important component of the clinical history of the patient. Any further clinical intervention the client confronts in life, will be met by these discharge summaries and interventions, of any nature will be determined by whatever it is the summary is telling the clinician.

I believe discharge summaries are critical to ensuring the client receives accurate and quality treatment. I personally have a horrible memory. In the moment and in the heat of a conversation, even when I tell myself to remember something that the other person said, something important, my memory fails me. When interacting with a client, we will not be able to recall every word, action or feeling they had hours or even days and weeks after. Thats why notes are so helpful, because it also allows for other team members to have access and stay up to date on the patient’s progress. They aren’t constantly trying to piece together the puzzles based off of something another employee told them. This ensures uniformity throughout the facility as well as different agencies they may be working with. The patients progress, treatment plans, addiction status are all necessary in a discharge summary. Treatment doesn’t stop just because a patient is discharged from a facility. The client will still be in recovery, they will still need assistance, encouragement and accountability and this protocol helps ensure that the client isn’t just left without a paddle once they leave.

This states that one must avoid dual relationships with clients. This covers bartering, social relationships, and sexual relationships. The client has all the rights to be treated with dignity and respect, As a counselor in the field we take the code of ethics to”DO NO Harm”. Due diligence tells us to work hard (in and outside our jobs) always at with this code in the forefront of our minds. As stated there is no room for ambiguity when it comes to sexual involvement with our clients. This is forbidden on all levels, for one to use what they deem as power is victimization of others. State licensing or certification boards impose significant punishment for counselors who violate this boundary, such as, license surrendered and payment of disciplinary costs, or License revoked; forever prohibited from holding a LPC or LMFT license in payment of disciplinary cost. Each State pose different sanctions, the bottom line is this, you will get terminated and any job you apply for you will be flagged licensed or not.
When a counselor violates a boundary such as sexual attraction/sexual involvement with a client it goes without saying, they themself have unresolved issues. This is why it is so important to be involved with clinical supervision and have a source outside of work that you can tap into, to process. Psychotherapy is a way to be able to release some of what one deals with in this field so one does not harm others and get burnted out.

In the Florida Certification Board (FCB) of Ethics, the common code of ethics violation is associated with not being able to set the right tone at the top. When addressing the issues related to drug abuse in common, the member of the FCB can be subjected to the challenges of failed use of the right tone. Public awareness, especially from the youth, can be associated with an increased desire to meet the goals within the shortest time possible. When things do not work as expected, the board of members can easily use the wrong tone to address the public (Jo et al., 2020). It might be considered arrogant and not caring about the integrity and value of the public image. Thus, any members are likely to violate the right tone that should be kept at the top.
However, various measures can be undertaken to ensure no violation of the code of ethics. The member can have enough preparation before meeting the public to discuss issues related to helping the youth and other individuals addicted to drug abuse. Proper preparations make it easier to evaluate the possible challenges that can be expected in public and affect the delivery of the right tone. Further, the preparations can help the members to identify the new ways that can be used to prevent the victims of drug abuse from getting resisting the intervention plans that can be put in place. The component helps build the right tone through public awareness that can ensure the violation of the tone is not experienced (Anzani, 2022). Thus, the can be frequent workshops for the members to allow them to learn more ways of implementing drug abuse policies in Florida without violating the board’s code of ethics.

The type of challenging client that has been or would be the most difficult for me based on my experience as a Recovering Addict in Recovery is dealing with newcomers in the program of Narcotics Anonymous. In fact, during the first 90 days of complete abstinence, a vast majority of recovering addicts give up and give in to the Disease of Addiction which is cunning, baffling, and powerful. During these challenging times it’s very important to attend meetings on a regular basics as well as share honestly about “thoughts of using”. Using this approach is the key element in overcoming not picking up that first drink or drug. It definitely worked for me during the early stages of my recovery which has enabled to remain clean and sober for the past 24 months One Day At A Time.

Which type of challenging client has been or would be most difficult for you to work with?
I do not work in the field yet I would imagine the most difficult type of clinic for me would be an elderly clinic that is dealing with the list of her husband whom she had been with most of her adult life. I can strongly feel others emotions. I also have no no experience with death. No one close to me has ever passed away. I have issues with death. In this case my empathy would be overwhelming. I have nothing to help to ease their pain. This is something that would crush my heart for them and I can do nothing to ease their pain. I would hate to see them grieve with no experience that could help them through it. I would feel powerless and it would be difficult to see them suffering due to their lost.
