Person centered care
Nursing Theory Concepts
Person-the recipient of nursing care, such as patients, families or communities
Environment-shifting the environment to change the patient’s health
Health-physical, emotional, intellectual and spiritual all affect patient’s health
Nursing-using knowledge obtained through education and practice for effective nursing care
(Black, 2019)
Nursing theory assists nurses in making reasonable judgements based on evidence, plan appropriate nursing interventions, and appraise outcomes of those interventions.
2
What is person centered care?
Understanding each patient is a unique person, regardless of culture, religion or community
Building a trusting relationship between patient and provider
Shifting thought process from disease-centered to holistic centered care
Goal is to improve individual health, and therefore improve community health as well

Disease centered care only focuses on curing the disease, does not necessarily take into consideration the patient’s culture or preferences. Shifting to a more holistic approach improves patient outcome and adherence to medical recommendations
3
Why is person centered care important?
Autonomy means the patient can make decisions for themselves independently, with the information that has been given to them.
4
Honors the nursing ethical code of autonomy
Allows more personalized care
Reduces unnecessary procedures
Improves patient outcomes
Compliance will be increased if patient’s feel they had their input
Examples of Patient Centered Care
Allowing patient to choose food options
Inviting family members to participate in decision making if the patient wishes
Letting patient choose when the time is best for education to occur
Allow time for naps if needed
Environment Factors
Noise
The main source in a hospital is people
Patient may experience sensory overload from notices.
Soft pleasant background music can mask other sounds and promote relaxation
Reduce noise by :
Avoiding long conversation in the hallway
Encourage staff to speak in lowered voice
Avoiding jokes and laughter at the nurses station
Answering alarm on IV pumps and equipment promptly
IMPROVING OUR INTELLECTUAL WELLNESS
IMPROVING OUR PHYSICAL WELLNESS
Health
Conversation
• Do you enjoy taking part in discussions, intellectual conversations, debates, or other ways of gaining an enhanced understanding of issues?
Befriend people who can stimulate your mind, and get into a discussion with them about topics that interest you
Sleep
Are you getting 7 to 8 hours of sleep a night
whenever possible?
• Are you avoiding exercise within a few hours
of bedtime, as well as “screen time”?
• Are you avoiding large meals before going
to bed?
• Try managing your routine to carve out time to rest and sleep, and cut back on caffeine.
