Program Design and the Logic Model
For this assignment, you will develop a logic model of your proposed program. The logic model is introduced in the Community Tool Box reading, Section 1. Developing a logic model or theory of change. A logic model is a valuable tool used to visually describe and communicate a program in a logical format that identifies goals, objectives, strategies, inputs, outputs, and impact. It is also useful for planning, implementing, and evaluating a program initiative. Building a logic model often helps stakeholders agree on a framework to assess the program’s overall effectiveness using clear and agreed upon criteria for evaluation. A logic model provides the “rationale for your program intervention and is often a required part of funding applications and program proposals. Use the file “DOC 1” attached to develop the program model.
Assignment Directions
Now that you have had an opportunity to reflect on your model program, this assignment will have you prepare a basic logic model that describes your program. You may use the Assignment Template (ATTACHED) and the Community Tool Box examples as your guide. These examples use boxes and arrows to form their model, but you may use any format or imagery that communicates your program more effectively.
Please addresses each of the following:
1) Create a visual logic model that investigates all the inputs (resources and investments), examines the outputs (activities, services, products, and participation), and analyzes the short and long-term outcomes, results, or impacts.
2) Include a brief narrative description of your model program that includes:
a) the purpose or mission of the program;
b) why the program is needed; and
c) what inputs (resources and investments), outputs (activities, services, products, and participation) will be used to achieve desired outputs (short and long-term outcomes), results, or impacts.
Assignment Guidelines
Your paper should include a completed logic model and a narrative that is no more than 2 pages, not including the title and reference pages, and should include the following elements:
Title Page: Provide your name, assignment title, course and section number, and date.
Body: Include an introductory and a concluding paragraph. Answer all the questions, reflecting appropriate personal and professional insights.
o The assignment must be written in Standard English and demonstrate superior organization, including a highly developed viewpoint and purpose that is clearly established and sustained. The communication of your thoughts must be highly ordered, logical, and unified, displaying exceptional content, organization, style, and mechanics (including the use of correct grammar, punctuation, and sentence structure).
o Your responses should reflect professional writing standards, using proper tone and language. The writing and writing style should be correct and accurate, and reflect knowledge of program development and administration.

o Include a minimum of two sources to support your responses and conclusions. One can be your text. The other should be from a peer-reviewed journal
Use Arial or Times New Roman 12-point font, double-spaced and left-aligned.
Use standard 1″ margins on all sides.
Use current APA formatting and citation style.
