As a public health professional, it is important to align public health programs with an organizational mission, vision, and objectives to ensure sustainability and growth within the target community.
As a group, review feedback received from your instructor from the Topic 3 assignment and make any necessary changes required to move forward with the project.
For this assignment, collaborate as a team and develop an action plan for the public health intervention you developed in Topic 3. Present your action plan in a 500-750-word paper and include the following:
Create a mission statement that aligns to the values of the public health intervention for the selected community/culture.
Draft a vision statement that concisely describes the overall goal for the public health intervention for the selected community/culture.
Develop three objectives that describe the measurable outcomes for the goal.
Outline the action steps for implementing the intervention.
Describe how continuous quality improvement (CQI) tools or techniques can be used to enhance this plan.
You are required to cite to three to five sources to complete this assignment. Sources must be published within the last 5 years and appropriate for the assignment criteria and public health content.
Prepare this assignment according to the guidelines found in the APA Style Guide, located in the Student Success Center.
This assignment uses a rubric. Please review the rubric prior to beginning the assignment to become familiar with the expectations for successful completion.
