The Problem Statement Paper

Your problem statement assignment will include thefollowing:

1. A brief, yet clear and explicit description of the organization (Hospital).
2. A description of the problem being observed or experienced in the organization (Hospital). Include the specific department and the context where the problem is being experienced. 
3. Who is being affected?
4.  What is the impact of the problem on the organization? Provide as many details to describe the problem that you can.
5. A single sentence that explicitly states the problem.

Problem Statement Examples: 

GOOD EXAMPLES:

• Hospital readmission rates are exceeding the benchmark set by Centers for Medicare & Medicaid Services (CMS).
• The hospitals HCAHP scores are 5% below established goals set by the administration.
• The adoption of EMR utilization is 30% when expected usage should be 90% at 6 months.
• Medication errors exceed documented limitations set in the policy (this may imply a clinical topic, but it has administrative ramifications for policies and procedures, staff development, the financial impact on the patient and the organization, and may even include issues related to HIT).

BAD EXAMPLES:

• Nurses are not washing their hands enough.
• Staff needs to be trained on the use of EMRs.
• Employees are disgruntled and unhappy.
• HCAHP scores are low.
