Please answer the following questions based off of the case study Chapter 16.3 in the Barnwell book.
How should a social worker support this individual? What are psychosocial stressors impacting this client? How might this client’s sexual identity be impacting his substance use? What other supports might this client need? What type of treatment could this client benefit from? What type of programs should he be referred to?
CRITICALLY RESPOND:
Julia- There are many things a social worker can do to support Mr. Vincent. For one thing, I think Mr. Vincent comes with many strengths, from having close friendships and a successful company, to also having the desire to seek out help for his substance use disorder, and I think a social worker can affirm these strengths. I think a social worker can start by doing a thorough assessment to get a sense of whether Mr. Vincent also has alcohol use disorder or stimulant use disorder, or any co-existing mental health disorders. Though I do think it’s important to do a thorough assessment, I also think it’s important for the social worker to understand that Mr. Vincent may well not have any other comorbidities. I think the social worker would ideally be someone who is themselves LGBTQ or is familiar with the LGBTQ community and the specific gay subculture that Mr. Vincent is a part of. If the social worker is not very familiar with this culture, I think they need to make sure they educate themselves about it and also check their own biases, especially biases that could lead to misdiagnosis.
One of the psychosocial stressors that may be impacting Mr. Vincent is peer pressure. Even if there isn’t explicit pressure to use substances, if others around him are doing it, it may feel normalized and expected of him. He is also looking for a life partner, so being single may be a source of stress for him. In fact, a lot of the activities where he is engaging in substance use are the party and play events, which often center on drugs and sex, and may be something he’s attendng while single, to meet people.
I think a social worker can do motivational interviewing to help get a clear sense of why Mr. Vincent wants to stop using cocaine and help him identify barriers to stopping. I also think cognitive behavioral therapy could be helpful. I think a peer support group or group therapy would be especially important for Mr. Vincent. He is not using these substances alone–it’s in a social setting, and it’s a normalized part of the social gatherings he’s attending. If there was a specific peer support group for gay men recovering from substance abuse that might be particularly helpful. It seems like he wants social connection and perhaps a support group would help provide that, and perhaps they’d have other suggestions of how he can have a robust sex life without using substances. Maybe there are substance-free party and plays for example, or maybe he could attend these party and play events with a couple of sober accountability buddies so that he can enjoy the party but also be there with others who are abstaining from alcohol and cocaine use.
Robles- Mr. Vincent is an individual that is a part of the LGBTQ community who is seeking treatment for his substance use disorder. It is important to assess Mr. Vincent in what strengths and weaknesses he may have that can help him through therapy. Assessing the individual also for other substance abuses such as alcohol and stimulants would be recommended. In this case Mr. Vincent does show signs of strength in that he has very close friendships and has maintained a successful company. The fact that the individual is reaching out for help is a sign that the individual may be ready to take control back over his substance use. Some psychosocial stressors can be that the individual may feel compelled to engage I these activities due to his peer interaction and trying to fit in. Also being a single man may be creating a sense of loneliness for the individual. This client’s sexual identity could have an impact on the client’s substance use as often individuals face many issues with society and trying to fit in however as client has stated he is very open of his sexuality and has no negative impact on his life. Forms of treatment can be individual or group therapy Which can include motivational interviewing used to understand why an individual wants to stop using substances and discussing specific barriers to treatment. Cognitive behavioral therapy can be used to help an individual learn about withdrawal, discuss thoughts, feelings and behaviors that lead to substance use, and to help create a plan to navigate triggers. Peer support programs That can help individuals commit to avoiding substance use and hold one another accountable and create social groups. I believe the individual would greatly benefit from any of the forms of treatment listed.
