Outline for Final Paper
Client Background
This section should include clients age and demographics, information about their
current living situation, employment status and place of employment, and brief information
about where they were born and raised, socioeconomic status and any cultural or social identities
they hold.
Past and Current Substance Use History
This section includes information about current and past substance use, any treatment
they have received, or attempts made to change their patterns. If they have been to treatment, this
section explains their experience there, types of therapy or treatment modalities they have used,
successes and challenges, any periods of sobriety, as well as relapse. This section discusses the
impact the substances have, and currently have on their daily functioning and any impairments.
Interpretative Summary
This section takes into consideration the impact of substances, the clients identities, daily
functioning, type of lifestyle and relationships client has. The goal of this section is to describe
the client taking into consideration their experiences and ways in which they have been impacted
and have formed their world accordingly.
Clients Co-Occurring Diagnosis
This section includes the diagnosis written out and formatted correctly, including severity
level and all specifiers. This includes justifying the diagnosis and ruling in and out symptoms
that explain why the client does or does not meet criteria for said diagnosis. In this you will
include information from the DSM-5 and any other information you may have from measures or
screenings used.

Corresponding Quadrant and Level of Care
This section will examine the Four Quadrants Model to classify care based on the
severity of the clients COD. The goal is to explain and justify which quadrant your client would
be placed in and consider the appropriate level of care that would be needed. You can use the
Quadrants of Care handout and Definition of Treatment Services handouts from the Asynch
material.
Family Systems Hypothesis
This section will examine the clients family system, including cultural identities and
possible barriers or strengths to success in treatment and recovery. This will expand on the
clients family members, children and close relationships, their history of substance use, history
of mental health concerns/diagnosis, and the impact this has had on the client. This will also
discuss support or lack of support the client has. Some of this information will need to be
hypothesized on.
Proposed Treatment Plan
This section discusses the appropriate treatment plan for the client based on your
conceptualization of them and all information provided. This section addresses the different
types of programs the client would benefit from, types of evidenced-based treatment and
modalities, and plans for after care. This section goes in depth about types of treatment, therapy
and can include housing options. For example, if the client were to start in an inpatient program,
move to a step down IOP and then be discharged to either a housing option in the community, or
within their family system.
