You will be able to copy and paste the preview of your survey in Survey Monkey into a Word document and use that for submission purposes in Canvas. Or, you can submit a word document in Canvas with a link to your survey if that’s easier! Here is a video tutorial on how to create a Web Link to your survey using Survey Monkey:

There is no minimum number of questions that you must have in your survey. Just be sure to create a comprehensive survey that touches on some key elements related to your organization!
