Walton et al. (2019) conducted a pilot study on the effect of a low carbohydrate ketogenic diet in newly diagnosed type 2 diabetes. The study recruited eleven women (mean BMI 36.3kg/m2) and HbA1c over 6.8% (mean 8.9%). The participants were subjected to an intensive dietary intervention restricting carbohydrates to less than 30 grams per day over 90 days. The primary outcome was HbA1c levels, and secondary outcomes were blood pressure, body weight, and blood lipids. The researchers ensured compliance through weekly plasma ketone tests. On evaluation after 90 days of intervention, the HbA1c mean dropped from 8.9% to 5.6%. In addition, there was a reduction in weight, systolic and diastolic pressure, and improvement in blood lipids. Thus, the diet was associated with reversal of the diabetes diagnosis and elicited beneficial changes in weight, blood lipids, and blood pressure in the short term. However, the study was limited to eleven highly motivated and self-selected women and limited to a three-month follow-up. Nonetheless, the study provides crucial insights into the benefits of the ketogenic diet in the management of diabetes type 2 with a significant reduction in HbA1c.
