Application Project
For this project, you should include the following information in your paper:
1. Definition of a community health related problem that a technology or informatics tool has the potential to solve/alleviate;
2. Description of the proposed technology or informatics tool;
3. Identification of key stakeholders;
4. Identification of key performance metrics by which to gauge success and a plan to monitor these metrics;
5. Description of a high-level implementation plan, including attention to communication, inter-professional collaboration, training, and desired outcomes;
6. Proposed changes to work flow that this technology/tool will introduce, as well as challenges that the technology/tool may present to end-users or to downstream departments;
7. A risk management plan to decrease the possibility of negative outcomes.
Your project should be no more than 4 pages excluding title page, (4) reference, and appendix. Submission by Blackboard, due 05/04, 11:59pm CST.
Please double-space your work and include a cover page, a title, and page number. You are encouraged to use subtitles in your paper to make organization of your paper clearer.
