Review the Module Five Health Education Activity Guidelines and Rubric document for this activity, as well as the Planner and Log and the Permission Letter. Remember: All planning work counts toward your eight hours!
I have attached the template, a student sample of the same, rubric, and my permission letter stating what I will be doing. I work with a community based agency dealing with clients with mental illness/substance abuse. The health education I chose to do is to teach the staff a review of mental illness, what it is etc, its described on the attached letter for your reference.
