Research requirement for a legal health record. Some places you may want to look at are AHIMA articles, Medicare’s COP’s, Accreditation Standards such as Joint Commission and NCQA, and Federal, State, and Local laws.

Respond to the following questions:

What is a legal health record?

What is contained in a legal health record?

What happens when the health record is part paper and part electronic (hybrid record)?

Are the requirements for a legal health record the same for all record formats? (paper, hybrid, and electronic)

Is the legal health record the same across the continuum of care? (think about acute care, physician office, nursing home, etc.)

Create a PowerPoint Presentation describing a legal health record and responds to the questions in Step Two. The PowerPoint should:

Contain slides not counting cover and reference slides.

Respond to each question.

Use in-text citations and references.

Make use of the Notes Section for speaking notes.

Contain appropriate font (at least 24 point font), background, use of graphics, and use of color.

NOTE: Do not include too much information on a slide – most of the information should be in the notes section with just highlights on the slide. Each slide should have no more than seven lines with seven words in a line.

Be audience ready (readable, no spelling/grammar errors).
