Discuss the elements of a prescription and the ways an APRN can protect his or her DEA number. Identify some of the most common prescription-writing errors and offer suggestions on how to prevent them.

There are many elements that must be included in a prescription: Prescriber information, provider information, inscription (includes the name and strength of the medication), subscription (dosage form and number of units or doses to dispense), signa or sig (take on an empty stomach or take with food; and how often to take), indication (why it is being prescribed, for example doxycycline for acne), refill information, generic substitution (or DAW “dispense as written”), child proof containers, warnings (if applicable – typically pharmacy will include this information), container information and lastly signature.
The FDA receives more than 100,000 U.S. annual reports associated with a suspected medication error. Our book discusses many errors that can occur when writing a prescription including: omissions – forgetting to include the drug name, strength, quantity to dispense, patient name, date, directions for how to take or prescriber’s name. Another error is not including the DEA number on a prescription with a controlled substance. Ways to prevent errors can be to write out everything slowly and legibly for example, when writing numbers such as when you are writing how many tablets you want to dispense you can write out “eight” instead of 8.
The privilege of prescription writing is a very serious task that should be responsibly handled and done meticulously. There are many common prescription errors that can and have occurred that can pose a threat to a patient. My sister is a pharmacist and she has told me many times how there has been time where the prescription was not legible and she had to call the provider to confirm the prescription. I don’t remember the exact medication but there was a patient who came in for their diabetes medication and instead almost got a medication GI related – thankfully my sister caught the other pharmacists mistake before it was dispensed. This is why we as prescribers need to be so careful when we write the prescription because even when it leaves our hands it still needs to go down the stream to the pharmacists who may misinterpret the prescription and dispense the wrong medication.
