Part 1 of the Final Project should be approximately 4 pages in length, written in essay format and include appropriate supporting citations and references in APA format. Create one reference page for both parts 1 and 2 of the Final Project.

Create a complete health promotion program plan.

Your final community-based health promotion program plan should:

•Clearly identify the target population and include a socio-demographic profile of the community.

•Identify the main health needs of the target population citing epidemiological data.

•Describe the health priorities that the program will focus on.

•Provide a rationale for the selection of health priorities based upon the results of a needs assessment that includes socio-economic factors, quality of life factors, and community capacity.

•Include an original program mission statement that clearly identifies the target population, program purpose, and scope.

•Include at least two (2) long-term program outcome goals.

•Include at least properly written three (3) program outcome objectives. You may choose to include a logic model that presents the program goals, objectives, and activities.

•Identify relevant program stakeholders.

•Include a sample budget for the program with an estimated total cost. This may be done in table form.

•Include a program planning and implementation timeline in table or flowchart form.

•Identify at least one potential source of funding and explain why the program is a good fit for this funder.

•Discuss one or more theories that will guide program development and implementation.

•Describe the strategies and activities the program will employ to achieve the stated outcomes.

•Provide a justification of how the activities are appropriate for the target population.

•Include a description of the program evaluation plan that includes at least three (3) specific evaluation questions.

•Discuss the types of data necessary to address the evaluation questions and describe the planned data collection methods and instruments.

•Describe the plan to implement the program. Include a discussion of staff roles, administrative tasks, and potential barriers to implementation.

•Conclude with a discussion of program sustainability.

Part 2: Create a workplace wellness program.

Part 2 of the Final Project should be approximately 4 pages in length, written in essay format and include appropriate supporting citations and references in APA format. Create one reference page for both parts 1 and 2 of the Final Project.

Create a workplace health plan for a corporation or organization. Include the following components in your program:

•Start with an introduction that orients the reader to the significance of workplace wellness.

•Identify the workplace in which you will implement the program.

•Discuss individual, organizational, and community factors that are relevant to health in the workplace you chose. Be sure to address demographics, socioeconomic factors, and health risks of the employee population.

•Create a program mission statement.

•Develop at least two program outcome goals and three program outcome objectives. You may choose to create a logic model to illustrate program goals, objectives, and activities.

•Create a sample budget for the program.

•Identify at least one potential source of funding and explain why your program is a good fit for this funder.

•Include a program timeline.

•Describe how the program will address program planning and workplace governance.

•Specifically address leadership support, management, the workplace health improvement plan, resources, and communication. Cite relevant literature and theory.

•Discuss program implementation to include a description of programs, policies, health benefits and environmental supports.

•Describe at least three activities the program will use to achieve the outcome objectives.

•Create an evaluation plan for the program that includes at least one evaluation question related to the program goals.

•Conclude your program plan with a discussion of program sustainability.
