A medication error is any preventable event that may cause or lead to inappropriate medication use or patient harm while the medication is in the control of the healthcare professional, patient or consumer (Hughes & Blegen, 2008). Medication errors occur in all settings and those with complex dosing regimens and given in specialty areas are associated with increased risks of adverse drug events. The target population will be healthcare professionals, including pharmacy staff, and administrative staff at the delivery unit of Southern New Hampshire Medical Center. There is a growing reliance on medication therapy as the primary intervention for most illnesses which exposes patients to potential harm when errors occur. The success of this initiative will ensure that patients only receive benefits of mediation through effective management of their illnesses and improved outcomes, with few if any errors (Hughes & Blegen, 2008). The initiative will also ensure that only evidence-based practices are employed to ensure patient safety.
Interprofessional collaboration will be required to implement this project. The high-performance and effective team will include registered nurses working on the floor, clinical supervisors, unit manager, physicians, pharmacy staff and pharmacist, director of education, director of research, and head of nursing services. This team will be involved during the entire process. Patients will be involved during the implementation and evaluation phases as their feedback will be valuable. The budget justification for this project will be based on the fact that healthcare today is value-based and cost-driven. Healthcare institutions are striving to provide the best care at the lowest cost. With reimbursement for quality of services provided, hospitals are wiling to pay the price to implement quality improvement projects that will ensure maximum reimbursement. Evaluation of the quality improvement initiative will be based upon the total number of medication administration errors made by nurses; the rate of the errors over the period of study; number and rate of errors related to high alert medications such as Pitocin, potassium chloride and magnesium sulfate; number and rate of errors related to omission, administration route, wrong drug, wrong patient and wrong time; the time of the occurrence and identification of the errors. The satisfaction rates of the patients and clinical staff (nurses, physicians and pharmacy staff) on the implemented initiative. Satisfaction rate will be measured in terms of how user friendly the process is, amount of time saved and how safe both nurses and patients feel during medication administration process.
