“Dizziness and balance issues are common concerns among the elderly. Dizziness, spinning, nausea, vomiting and balance issues can represent several different health concerns in the elderly population. It is essential for proper diagnosis, treatment plan, testing, referrals and education. When the diagnosis is benign paroxysmal positional vertigo treatment plan is greatly different from a TIA.
The PCP office I am currently at we commonly have compliants of dizziness, spinning, with and without nausea and vomiting and balance disturbance. Proper history and examination allows for the proper diagnosis. Many of the these patients are elderly patients. The 3 differential diagnosis would be BPPV, orthostatic hypotension, and dehydration. AFTER ruling out TIA or stroke. The use of dix-hallpike is used for diagnosis of BPPV (Nahm, Hyunjoo, Han, Kyujin, Shin, Jung & Kim, Change, 201. ). One treatment option is canalith repositioning maneuver (Nahm et al, 2019). The PCP office I am will refer to physical therapy for vestibular rehab as well. BPPV is the most common cause of peripheral vertigo in the elderly (Nahm et al, 2019). It may be more difficult to treat in the elderly in comparison to younger counterparts who suffer from BPPV ( Nahm et al, 2019). The elderly are also at increased risk from falls due to vertigo and may suffer more significant problems such as broken bones or increased bleeding (already on blood thinners) (Iwasaki &Yamasoba, 2015.”
