This case study describes a patient that was recently intubated with decreased oxygen (02) saturations and signs of respiratory failure. The intubation appeared to be successful based on visualization of the tube passing through the vocal cords, confirmatory chest rise during manual ventilation and normal end tidal C02 reading. In evaluating the CXR below that was obtained immediately after the procedure, it is evident that the patient has a total white out on the (L) side. This indicates that the tube has entered the (R) main stem bronchus and the (L) lung has become atlectatic. Therefore, the tube must be pulled back to the level of the carina.
