71 years old female presents with acute lower backpain onset one-week, constant dull ache. States it isworse with lifting or moving anything and is painfulto walk; relief of pain with laying down on back. Hxof hypertension, hyperlipidemia, andosteopenia. Denies any fever, weight loss, numbness,tingling or problems with urination. Tylenol has not helped. Denies any recent trauma, accidents, or surgeries.
Diagnostics:
- Bone scan, total body: Normal.
Lumbar spine MRI: Anterior wedge-shaped fracture of L4.
- Spine x-ray: Anterior wedge-shaped fracture of L4.
Thoracic spine MRI: Normal.
- Referrals/Consultations:
Referral for physical therapy and potentially pain management if her pain becomes
- uncontrollable.
Referral for surgical consult for possible vertebroplasty (last resort)
- Medications/Prescriptions/Treatments:
NSAIDS first-line treatment alternating between Tylenol and ibuprofen.
- Alternate between hot and cold compresses.
Ibuprofen 800mg q8H. Tylenol 650mg q8H
Client Education
Wear a back brace.
- Alternate between hot and cold compresses.
Use NSAIDs as prescribed.
Avoid heavy lifting.
Follow instructions given by PT.
