Please apply if you can access the readings. there should be 3 or more citations with reference Cardiovascular Conditions CASE STUDY: CHEST PAIN Patient is a 62-year-old male visited the clinic stating, “I’ve been having pain in my chest off and on”. “I think it might be indigestion but still want it checked out”. It started 2 weeks ago about 10 to 15 minutes when raking leaves and tries to ignore it but will not go away. He rates his pain at 5 or 6. Got better when stopped and rested for about 10 minutes. The pain feels “heavy like there is a weight on it”. Denies pain radiating to shoulder or arm but it sometimes feels like a squeeze to the throat. The pain is worst with activity. It occurs every 3-4 times a week. His has past medical history of hypertension and hyperlipidemia. He is worried because he recently lost his job. THE ASSIGNMENT TO BE COMPLETED Write up the a good HPI for this case study. Provide evidence from the literature to support diagnostic tests that would be appropriate for differential diagnosis. List five different possible conditions for the patient’s differential diagnosis. Your primary diagnosis should be at the top of the list. For each diagnosis, provide supportive documentation with evidence-based guidelines. Justify why you selected each. Create management plan for your primary diagnosis which includes pharmacological and nonpharmacological, patient education. Support your work with least three different references within the last five years (one or two reference from current evidence-based literature from your search and one or two different references from this week’s Learning Resources).
